Welcome to Abicare Hospital@Home, supported by Healix Health. We aim to brings safe, expert hospital care into your home. We work in partnership with your local NHS hospital to help you leave hospital sooner and continue your recovery where you feel most comfortable – at home.

This approach supports the vision set out by NHS England to move more care closer to where people live. It means receiving the right care, in the right place, at the right time – without staying in hospital longer than necessary.

With over 20 years of healthcare experience and more than 5 million hours of care delivered, our nurses, physiotherapists and healthcare support workers provide personalised, one-to-one care – all with the oversight of your GP and hospital team.

How Does Hospital@Home Work?

Identifying the right time

Your hospital team will confirm when you are medically fit to leave hospital but still need short-term clinical support.

Planning your return home

We work closely with the hospital to:

  • Review your medical needs

  • Agree your suitability for home care

  • Arrange your discharge

  • Organise transport home

A nurse visits within 2 hours

Your lead nurse will arrive shortly after you return home. They will:

  • Assess your home environment

  • Review your medical information

  • Create your personalised care plan

  • Arrange nursing, physiotherapy or healthcare support visits

  • Set up remote monitoring if required

Ongoing expert support

From the very next day, your care begins.


On average, patients receive around 1–2 hours of care per day from our combined clinical team, depending on individual needs.

You will also have

Ongoing Clinical oversight

Regular reviews and adjustments to your care plan

We can provide up to 6 weeks of care, with options for continued support if needed.

Is this safe?

It’s completely natural to have questions about receiving hospital care at home.

We have already demonstrated that this model of care can be delivered safely, efficiently and with excellent outcomes. Every patient accepted into the service is medically stable and carefully assessed. Your care is delivered by trained clinical professionals, supported by technology and close coordination with your hospital and GP.

This approach is becoming an essential part of building a more sustainable and patient-centred healthcare system – keeping people safely out of hospital wherever possible, while maintaining high clinical standards.

Who pays for this service?

There is no cost to you.

Your hospital and local Integrated Care Board fund the full cost of your care.

What types of patients can we support?

We support patients who are medically stable but require short-term clinical care at home, including:

  • Frailty and falls recovery

  • Pneumonia (including IV antibiotics or oxygen therapy)

  • Heart failure requiring IV treatment or monitoring

  • COPD exacerbations

  • Cellulitis requiring IV antibiotics

  • Post-operative wound care

  • Rehabilitation after surgery

  • Monitoring following an acute illness

If you are suitable, your hospital team will discuss this option with you.
