  • Regular clinical reviews

  • Access to a 24/7 nurse-led urgent support line

  • Oversight from your GP and hospital team

If your condition changes, we act quickly and coordinate with your hospital if needed.

You will receive the clinical care you need, delivered safely at home.

Your care may include:

  • Nurse visits

  • Physiotherapy

  • Healthcare support worker visits

  • Medication support (including IV therapy where appropriate)

  • Remote monitoring, if required

Your care plan is tailored specifically to you.

A lead nurse will visit you within 2 hours of arriving home.

Your full care package will begin from the next day, based on your individual needs.

You will have access to our 24/7 nurse-led urgent support line.

If you are concerned at any time, you can call and speak to a qualified nurse who will assess the situation and advise you on the next steps.

Hospital@Home provides care for up to 6 weeks, depending on your recovery needs.

If you require ongoing support after this period, we can help arrange continued care through our sister service.

No. There are no charges to you.

Your hospital and local Integrated Care Board fully fund the service.

Absolutely.

Recovering at home allows family, friends and carers to be part of your recovery in a natural and supportive way. Our team will also involve them (with your consent) in understanding your care plan.

Before your care begins, our lead nurse will assess your home environment to ensure it is safe and appropriate.

If any adjustments or equipment are needed, we will discuss this with you and help arrange suitable solutions.

We commonly support patients recovering from:

  • Pneumonia

  • Heart failure

  • COPD exacerbations

  • Cellulitis requiring IV antibiotics

  • Post-operative recovery

  • Falls and frailty-related admissions
